Trent Hill Center is seeking a Case Manager for our new Assessment Center, opening in fall 2025. This role will work hand in hand with the Program Manager, Educational Coordinator, Direct Care staff and Clinical Director to ensure that our participants’ immediate and longer-term needs are being addressed.
General responsibilities of the Case Manager include but are not limited to:
· Conducting initial intake of Assessment Center participants to discuss their immediate and long-term needs and goals.
· Coordinating medical, dental, vision and other appointments for participants when needed, and following up on results and prescribed medications.
· Referring participantsfor mental health services as needed.
· Communicating with DSS about issues with participant placement, behavior, or other needs.
· Collaborating withProgram Manager, Educational Coordinator and others as needed to create serviceplans and goals for participants.
· Monitoring participant progress within THC programming and outside programming as applicable.
· Working with participants to identify additional family members or friends who can provide support for the participant.
· Documenting all participant progress for their case records.
· Uploading clinical information into electronic portal, as required by the Center’s licensing and contracting standards.
· Administrative tasks as needed.
Standard Responsibilities:
· Respect and maintain the confidentiality of youth participating in all Trent Hill Center programs.
· Report to work on time and on a regular basis.
· Always conduct oneself in an ethical manner.
· Treat participants and other employees with integrity and respect.
· Report suspected child abuse and neglect to the Out of Home Abuse and Neglect Unit of SCDSS or to a law enforcement agency in the county where the child resides or is found.
The ideal candidate will have experience working with foster care youth and have some case management experience. Bachelor’s Degree is required. Staff should be at least 21 years of age. This position will report to the Clinical Director of the Assessment Center.
The Case Manager is in a full-time, salaried position.
